Boarding Contract

    I, being at least 18 years of age, the legal owner or one of the legal owners of my "Pet(s)" listed below, or the authorized agent acting on behalf of the Pet's legal owner (the "Owner"), do herby entrust Back Road Boarding, LLC (herein referred to as "BRB") to care for my Pet(s), during their boarding stay during all dates requested, consent to my Pet(s) medical treatment as stated herein and, as the Owner, do herby agree to all the terms listed below and represent and warrant that all of the statements below are true, accurate, and correct. I understand that this agreement creates binding legal obligations between the owner and BRB and I have read it carefully.

    1. Boarding Charges
    I agree to pay the daily boarding rate for my Pet(s) in addition to any extra services or products requested by me at the end of my Pet's stay in the form of cash, check, or credit. I have been fully informed and understand the posted hours that I can drop off and pick up my Pet(s). I understand that, if I drop my Pet(s) off during BRB's am hours or pick up during BRB's pm hours I will be charged a full day rate and if I drop off my Pet(s) during BRB's pm hours or pick up during BRB's am hours I will be charged a half day rate. I am also aware if I call ahead and request a drop off or pick up time outside BRB's normal business hours and the request is accepted I will be charged a full day rate plus an additional fee. I am also aware that BRB is not staffed 24/7 and if I arrive on premises outside of BRB's normal business hours without calling ahead a staff member may not be available to assist me upon my arrival; however, if a staff member is available I will be charged a full day plus a larger additional fee. I understand and respect that the owner of BRB lives on premises and I agree to respect the business hours established by the owner.

    2. Intact Dogs
    I understand that BRB accepts both intact female and male dogs. I further understand that if my Pet(s) remain intact they may behave differently in a kennel environment as a result. I understand and agree that BRB requires intact male dogs over one year of age be housed individually in separate kennels. I understand and agree that BRB allows intact dogs of opposite genders to be housed together, however BRB cannot be held responsible for any litters conceived in their care.

    3. Food & Medication
    I understand that I am responsible to provide BRB with an adequate supply of my Pet(s) food and any medication. BRB recommends providing additional supplies in case of an emergency requiring extended care. Should the food/medication need replacement, I authorize BRB to purchase replacement and I agree to reimburse BRB for the cost plus a replacement fee. I understand that if I request my Pets to be boarded together BRB will assume my Pets can be fed together unless I specify otherwise. BRB is not responsible for monitoring food and water each individual Pet consumes within a shared kennel. I further understand Pets that need fed separately must be kenneled separately for meals, but can be boarded together during non feeding times, however I will be charged accordingly. BRB provides stainless steel food bowls and water buckets.

    4. Belongings
    I understand that I am responsible to provide BRB with any bedding and toys I would like my Pet(s) to have in my Pet's individual kennel. I understand that I am responsible for my Pet's belongings. BRB will take necessary precautions to remove damaged belongings; however, BRB cannot be held responsible for any consequences that result from damaged and consumed belongings. I further understand BRB is not responsible for any lost or damaged items and recommends clients provide their Pet(s) with old blankets and/or towels.

    5. Playtime
    I understand and agree that my Pet(s) will have individual access to one of BRB's 5 fenced in areas a minimum of 4 times per day. I understand that my Pet(s) will not participate in Group Play at BRB unless I am present to supervisor my Pet(s). BRB will take reasonable measures that my Pet(s) is/are contained within the fenced area, but does not assume any liability in the event my Pet(s) escape the confined area. For the health and safety of your Pet(s) please note below if your dog has a history of jumping or climbing fences and height of said fences as well as if your Pet(s) has/have a tendency to dig.

    6. Vaccinations and Conditions
    I affirm that my Pet(s) has/have had all of the vaccination required by BRB (Rabies, Distemper, & Bordetella) from his/her veterinarian and I understand that BRB requires documented proof of said vaccinations prior to boarding my Pet(s). I understand that BRB, in its sole judgment and discretion, will take all necessary precautions and actions with respect to the health and well being of my Pet(s). I understand that my Pet(s) could become infected with Canine Influenza Virus (dog flu) or Tracheobronchitis (canine cough), or such other airborne viruses, during his/her stay with BRB. I affirm my Pet(s) has/have been vaccinated by my Veterinarian for these viruses according to their protocol, and I will not hold BRB responsible for the cost incurred for such treatment if my Pet(s) develop these viruses. I will seek advice from my veterinarian if either of these viruses develop. I further understand that, should my Pet(s) have any physical or behavioral conditions that may cause him/her hazard to itself, or another animal or person that I have a legal obligation to disclose this information to BRB prior to my Pet's stay.

    7. Medical Decisions
    I authorize BRB to do whatever they deem necessary, in their sole judgment and discretion, for the health and well-being of my Pet(s) and agree to pay for any and all expenses relating to his/her care, or to reimburse BRB for such costs it incurs in the course of caring for my Pet(s). If my Pet(s) should require medical attention while in their care, I understand that BRB will attempt to contact me if time permits, to inform me of the condition, and also attempt to contact my Veterinarian to schedule an appointment. However, should the nature of my Pet's illness require an office visit, I understand that BRB may take my Pet to whatever veterinarian it chooses in its sole discretion.

    IF MYSELF, OR MY EMERGENCY CONTACT ARE UNREACHABLE, I GIVE MY CONSENT TO AND AUTHORIZE BRB TO ACT AS MY AGENT AND TO SEEK CARE AND MEDICAL DECISIONS FOR MY PET(S) ACCORDING TO BRB'S SOLE JUDGMENT AND DISCRETION IN CONSULTATION WITH A LICENSED VETERINARIAN, AND I AGREE TO PAY FOR THESE SERVICES FOR MY PET(S) OR REIMBURSE BRB FOR SUCH COSTS.

    8. Release of Liability and Indemnification; Limitation of Liability
    To the fullest extent permitted by law, I release BRB and its owners, officers, directors, employees, agents and representatives (the "Back Road Boarding Parties") from any injury, harm, damage or death which may occur to my Pet(s) and agree to release and hold harmless the BRB Parties from and against any and all claims, liabilities, suits, actions, administrative proceedings, costs, fines, losses, penalties and damages, including, without limitation, reasonable attorneys' fees (collectively, "Damages") related to my Pet's stay with BRB. I agree to indemnify, defend and hold harmless the BRB Parties from and against any Damages to another pet, person, or property related to my Pet's stay with BRB, except in cases of gross negligence or willful misconduct by BRB. Notwithstanding the foregoing, in the event that BRB is held liable for any damages, I agree that their liability shall not exceed the total amount I have paid BRB in the current calendar year.

    9. Abandonment
    I understand that BRB will not assume responsibility of my Pet(s) beyond the agreed upon length of stay. If I do not contact BRB or cannot be contacted by BRB concerning an extended stay for my Pet(s), BRB will maintain responsibility of said Pet(s) for five consecutive days before contacting a Pennsylvania Dog Warden.
